Dr Joseph Smith was undergoing training at Biggleswade Hospital and Bedford Hospital in 2018. The training meant that he had to work under supervision with feedback being completed at each shift. The feedback form was to include any cases he dealt with, any learning areas and competencies in discussion with his supervisor at a shift in March 2019. He did not attend that meeting.
The supervisor later found out that the form had been filled in, including her name, and contained a forged signature.
Dr Smith denied doing the form, and questioned the credibility of his supervisor when challenged. He admitted the allegation at tribunal along with another charge.
The Medical Practitioners Tribunal Service (MPTS) found the two allegations proven. Dr Smith was suspended for five months with a review hearing directed.
